Can you play midis on your PC? If you can, then the SW Synth volume is turned down low or muted, increase the SW Synth volume or unmute it.
EDIT: Have you updated your sound drivers?

Maybe he's on Vista.
I got the problem that I can hear shitty "Microsoft GS Wavetable Synth" music *shivers* in the XDuke version, but when I start eDuke the music on/off switch in the menu is gray and I can't hear any music.
It's a Vista problem on my side I guess. I can't load any better sounding Midi libraries besides that crap Microsoft thing either because it's limited/prohibited driver-wise.
I'm running Vista 64 and a Creative X-Fi Extreme Audio PCIe with actual drivers.

What OS do you have? Do you have more than one soundcard? I remember having troubles with music with a SB Audigy and motherboard soundcard both activated.
