To use the portal wrench, you have to use the alt-fire button (default is RMB: if you redefined RMB as "forward", you will need another key for alt-fire) while you're holding the wrench. You press alt-fire once and it will look like nothing happened, but you just marked the spot for the first portal. Walk elsewhere, press alt-fire again (marking the spot for the second portal) and both portals will open. If you press alt-fire a third time, both portals will close.
